Does Dr. Smith take my insurance?

  • Dr. Smith is a network provider for insurance plans affiliated with BSA Provider Network, Blue Cross Blue Shield, Aetna, IMS, United Healthcare, Firstcare, Medicare, and many others. Check your insurance directory for in-network providers as your benefits will be substantially different if we are an out-of-network provider. You can call us with any specific insurance questions you may have.

What will I need to bring to my first appointment?

​

  • For your first appointment or if you are an existing patient who has not had an appointment in over a year; you will need to bring your insurance card, your driver’s license, and a list of all current medications, and we will need to take your picture when you check-in. As a new patient or an existing patient with a new complaint, you will need to fill out the new patient forms. This may be done through our website (here) or may be printed and brought to your appointment.

How soon can I make an appointment?

  • If you would like to schedule an appointment please call our office at 806-359-0718, and we will be happy to schedule your appointment as quickly as possible.

  • No, our office offers a variety of treatment options including physical therapy, injections, and surgery. Dr. Smith will assess and decide which route suits your problem and needs.
