What will I need to bring to my first appointment?

For your first appointment or if you are an existing patient who has not had an appointment in over a year; you will need to bring your insurance card, your driver’s license, and a list of all current medications, and we will need to take your picture when you check-in. As a new patient or an existing patient with a new complaint, you will need to fill out the new patient forms. This may be done through our website (here) or may be printed and brought to your appointment.
