Kim's Comments on the office
Welcome to Dr. Smith's office. Our office hours are Monday through Friday from 8:00 am to 5:00 p.m., except for holidays. We are normally closed from 12:00 to 1:00 p.m. for lunch. We do have an answering service that takes our calls when we are not in the office, and we also have a voice mail system that you can leave us a message and we will call you back as soon as possible. Dr. Smith has appointments available on Monday and Wednesday from 8:30 am - 4:30 p.m. He is in surgery on Tuesday and Thursday. Of course with him only being here only two days a week really plays havoc with our schedule, but we will try to accommodate you as best as we can. We are currently accepting new patients. Some insurance plans do require a referral from a treating physician, ex: family doctor or internist, to a specialist. If another Orthopaedic surgeon has previously seen you, we may require a referral from that physician. We try to be as accommodating as possible and if you have any questions or problems with an appointment time, please feel free to call me. We usually try to send our new patient forms to you in the mail for you to fill out and bring back with you to your appointment. To make your appointment go as smooth as possible, we do ask that you to bring all of your medical records, diagnostic tests (MRI, nerve test, arthrogram, etc.), or x-rays that pertain to the problem we are seeing you for. We are currently linked to the Baptist-St. Anthony's Hospital by computer and can review any x-rays that were taken at the hospital, but we still need the reports. I do realize that it is not always convenient to retrieve all of this information, but it helps Dr. Smith figure out what has been done and what he can add to that to help you. We do ask that you bring your driver's license, social security card, and your current insurance card with you on your initial visit so that we may make a copy of them for your file.
